Transition Services & Resources

What is Transition?

Transition planning is helping students with disabilities and their families think about their lives after high school. Planning also helps identify long-range goals and designing the high school experience to ensure that students gain the skills and connections they need to achieve these goals.

The Texas Education Agency, in collaboration with Statewide Leadership in Secondary Transition and Post School Results at Education Service Center Region, are pleased to announce that the Texas Transition and Employment Guide has been posted to the Transition in Texas Website.

There is also direct link to the Guide here. The purpose of this guide is to provide information on statewide services and programs that assist students with disabilities in the transition to life outside of the public school system.

Transition is an IDEA mandated Special Education program available to students ages 18-22 who have met graduation requirements and continue to work toward individual goals in deficit areas.
