Aledo ISD follows the Texas Department of State Health Services (DSHS) immunization requirements for school attendance. Families should provide current immunization records at enrollment and keep records updated whenever additional vaccines are received.


Texas School Immunization Requirements

Texas law requires students attending public schools to have documentation showing they meet the state’s immunization requirements, qualify for provisional enrollment, or have a valid exemption on file. DSHS publishes the official minimum vaccine requirements for students in grades K–12, and those requirements are the standard Aledo ISD uses for enrollment.

Acceptable Immunization Records

Students must submit acceptable immunization documentation before entry, attendance, or transfer, unless they qualify for provisional enrollment or have an approved exemption. Texas DSHS also recognizes certain alternatives in specific situations, such as documentation of prior varicella illness in place of varicella vaccine.


Provisional Enrollment

A student may be enrolled provisionally when allowed under Texas rules, such as when the student has started the required vaccine series and is continuing to receive doses as quickly as medically feasible. However, a student who does not have required immunization documentation, is not progressing through needed doses, and does not have a valid exemption may not continue to attend school.


Immunization Exemptions in Texas

Texas allows school immunization exemptions in limited circumstances, including:

  • Medical exemption
  • Reasons of conscience, including religious belief
  • Active duty military exception in certain circumstances under Texas rules

Medical Exemption

For a medical exemption, the school must receive a written statement from a physician stating that a medical reason exists that prevents the student from receiving specific vaccines. Unless the statement says the condition is lifelong, the exemption is valid only for the time specified by the physician.


Texas Immunization Exemption Affidavit

(Reasons of Conscience, Including Religious Belief)

Parents or legal guardians requesting an exemption for reasons of conscience must use the official Texas DSHS affidavit form. Texas DSHS now makes the blank affidavit available online for download, and families may also request that DSHS mail the form. The affidavit must be completed, notarized, and submitted to the school. It may not be altered before submission.

Important details for families:

  • The affidavit must be the official DSHS form.
  • The affidavit must be notarized before it is submitted to the school.
  • The affidavit is valid for two years from the notarized date.
  • The affidavit must be submitted to the school within 90 days of notarization.
  • The notarized original must be kept on file at the school.
  • Students with a conscientious exemption may be excluded from school during an emergency or epidemic declared by the Texas Commissioner of Health.

Parker County Hospital District Outreach Program offers immunizations at the Outreach Center located at 1115 Pecan Drive, Weatherford, Texas 76086.  Immunization services are provided Monday through Friday from 8 AM to 4 PM - appointments are not necessary.  You can reach them at (817) 458-3254.

Vaccines are also available through your child’s primary care provider.
